           I, Sarah Livingston, being duly            sworn,    depose and state as follows:

                                                                               LLC    ("ProVest"), the agent retained by the attorney or   law firm
                 1.    I   am an   employee of ProVest Services
                       representing the      plaintiffin the above captioned action.

                 2.    I have not beenprovided with a social security number for JANE DOE (N/K/A CLAUDIA VALENCIA)
                       ("Defendant") nor have I been able to locatea social security number for the Defendant by searchingindustry
                       standard databasesavailable to me. Without a social security number for the Defendant I am unable to search
                       the data banks of the Department of Defense Manpower Data Center and obtain a Status Report Pursuant to
                       Servicemembers Civil Relief Act pertaining to the Defendant. Therefore, I am unable to determine whether
                       the Defendant is or is not on active duty in any of the Uniformed Services of the United States of America.
