of the defendant(s) alleges as follows:
1. That the plaintiff is a domestic corporation authorized to transact
business in the state of New York. Plaintiff is licensed by the NYS
Banking Dept as such is not required to be licensed by the NYC Consumer
Affairs Dept.
2. That the defendant(s) resides in the county in which this action is
brought or that the defendant(s) transacted business within the county
in which this action is brought in person or through his agent and that
the instant cause of action arose out of said transaction. Plaintiff is
the original creditor. The contract was given account number
ending in 5977
3. Plaintiff is the holder, for value of a retail installment contract dated
03/04/23 duly excuted at QUEENS AUTO MALL 22 134-01 ATLANTIC AVE
RICHMOND HILL NY 11418 and delivered and/or guaranteed by the defendant(s).
4. Upon information and belief, defendant(s) defaulted in making the payments
due therein.
5. Defendant last paid $431.77 on 04/04/23.
