to or can reasonably be ascertained by petitioner, are:
1 Decedent. The above named decedent (date of birth: January 24, 1973) who resided at
2097 Friendship Road, Waldron, AR 72958, died intestate in Waldron, AR, on June 6,
2023 (See Death Certificate attached as Exhibit A).
Surviving Spouse, Heirs and Devisees. The surviving spouse, heirs, and devisees of the
decedent, and their respective ages, relationships to the decedent, and residence

Person to be appointed. Petitioner nominates herself, Wendy Smith, whose address is
2097 Friendship Road, Waldron, AR 72958, for appointment as Administratrix. The
relationship, if any, of the nominee to the decedent, and other facts, if any, which entitles
the nominee to appointment is that she is familiar with the deceased and is the mother of
the decedent and best suited heir to assume the role of administratrix.
WHEREFORE, petitioner prays that this Court enter an Order determining the
fact of the death of the decedent and of the intestacy of the decedent, and appointing the
above nominee as Administratrix of the estate.
