Copies of all medical or psychological reports of allphysicians and other medical —--——----- - providers who have treated or have-examined the but not limited to the plaintiffpincluding reports of allphysicians and other medical or psychological providers who will testify for the plaintiff. Duly executed and acknowledged written authorizations permitting all parties to obtain and make copies of all treatment records and such other records, as may be referred to and identified in the reports of those medical providers who have treated or examined the plaintiff. We note that plaintiff has previously provided authorization for the records of Dr. Niarcia Westra, which was forwarded to Dr. Westra, and Dr. Westra has provided to the Orangetown Defendants notes relating to her treatment of Plaintiff Phadia Jean for the time period from January 13, 2106 until March 28, 2018. However, Plaintiff in her EBT testified that she began treatment with Dr. Westra in August 2015. Defendants specifically request another authorization permitting release of Dr. Westra's notes from August 1, 2015 until January 12, 2016. PLEASE TAKE FURTHER NOTICE that this is a continuing demand.
