Plaintiff or Co-Defendant.

and/or have examined Plaintiff, including but not limited to, Plaintiff's treating
physicians and/or all physicians and/or other health care providers that have
examined and/or reviewed Plaintiff's medical records on behalf of the Bureau of
Workers’ Compensation

diagnostic testing of Plaintiff.
