IG I hereby make an election under Section 318.14(5), Florida Statutes, to have a civil traffic
hearing before the Judge. I understand that by making this election I am giving up the choice to attend a
driver improvement course. I further understand that if the Judge determines that a civil infraction has
been committed, the Judge could impose a civil penalty not to exceed $500.00 (except that in cases
involving unlawful speed in a school zone or unlawful speed in a construction zone. In which case, the
civil penalty may not exceed $1,000.00), plus costs of $25.00, or require attendance at a driver
improvement school, or both.
