billed to Defendant in this action are the usual and customary charges by Plaintiff for the
services rendered and Deponent believes them all to be reasonable and properly charged;
that this action was commericed by service of a Summons and Complaint upon Defendant;
that the time of the Defendant to appear or Answer herein has expired and the Defendant has
not appeared or Answered herein.
WHEREFORE, Plaintiff demands Judgment against the Defendant in the sum of
