Date of Death: July 18, 2022
L CASEY R. FLACU, after first being duly sworn, do depose and say that:
1. Iam the duly appointed and qualified Personal Representative of the Estate of the
Decedent filed with the Berkshire County Probate Court Docket No. BE22P0649.
At the time of his death, the Decedent owned an interest in real estate situated at 5
Christian Hill Road, Great Barrington, Berkshire County, Massachusetts, as more
particularly described in a deed from Nancy Reardon Flach, dated March 4, 2009
and recorded on March 11, 2009 in the Southern Berkshire Registry of Deeds in
Book 1895, Page 323. ,
The gross estate of the decedent does not necessitate a Massachusetts estate tax
filing, nor a Federal estate tax filing.
This affidavit is given pursuant to and in accordance with the provisions of
Massachusetts General Laws Chapter 65C, Section 14 (a).
