              I request the following records from the DCWP under NY Pub. Off. L §§ 84 et seq.:

                  1. A copy of any survey referenced in DCWP’s June 14, 2022 Press Release
                     available at https://www1.nyc.gov/site/dca/media/pr061422-DCWP-Holds-
                     Delivery-Worker-Public-Hearing.page (hereinafter, the “Survey”).
                        a. To the extent that multiple surveys are used (to account for device type,
                            language, or any other variable), this request encompasses all such
                            surveys.

                  2. All records explaining or showing how the list of recipients of the Survey was
                     generated. (We do not seek records showing the contact information of the
                     recipients.)

                  3. All responses to the Survey (including any responses DCWP receives going
                     forward).

                  4. All records showing:
                         a. How many email or phone numbers the survey link was sent to,
                         b. How many individuals the survey link was sent to,
                         c. How many survey responses were received, and
                         d. All other records showing or pertaining to the response rate to the survey.
