Upon information and belief that during the time heretofore at all times hereinafter
mentioned, the defendant(s) herein were and still are residents of the State of NEW YORK.

That the plaintiff(s) issued a credit card to the defendant(s).
That the defendant(s) used such credit card and incurred charges for the agreed

and reasonable sum and balance due of $5967.37.
(6)
No part of said sum has been paid although duly demanded.
6) That plaintiff mailed to the defendant a statement of account in the sum of $5967.37
on or before 02/18/2016. Defendant has retained same without objection thereto resulting in an
account stated.
WHEREFORE, plaintiff(s) demands judgment against 5said defendant(s) for the
