                        DIOLET SIMEXAN, being duly sworn, deposes and says under penalties of perjury that:

                        On May 27, 2021, between 8:00 am and 9:00 am I was driving West on Central Avenue

                in Monsey, NY. The weather was clear, and the roads were dry. West Central Avenue is a two-

                way roadway for East and westbound traffic. It is wide enough for one lane of moving traffic in

                each direction and no parking lanes on either side. I was coming from Monsey Boulevard

                traveling eastbound on West Central Avenue.    I was driving behind an SUV and there was a

                yellow school bus in front of us. The school bus stopped, put on its flashing yellow lights and

                opened the stop sign. There were 10 or more children crossing the street to go to the bus. The

                last child was crossing the street to get to the bus when the SUV went around the school bus and

                came into contact with the young child. The driver of the SUV was rushing and hit the child. I

                immediately blocked the road with my car to prevent other vehicles from passing. Someone

                called 911. A Jewish ambulance came to the scene right away. The child was laying on the street

                and was bleeding from the leg. The child’s leg looked broken. The child was crying a lot. There

                were a lot of people who came outside. The police also came to the scene very fast. I spoke with

                the police and told them what I witnessed. I have read the above and it is true to the best of my

                knowledge.
