                                       I, Brandon J. Mika, Esq., declare and state as follows:

                            attorney for the law firm, McCaiihy & Holthus, LLP, attorneys of record for Plaintiff Complete

                            Business Solutions Group, Inc dba Par Funding ("CBSG")

                                       3.           On or about November 8, 2019, my office filed an application for entry of sister-

                            state judgment in the above-captioned matter, which was entered by the Court on November 13,
