Montefiore Medical Center Emergency Room records dated 05/02/2018. 2. Medical records from Central Park Physical Medicine & Rehabilitation. 3. MRI scans from Kolb Radiology of the right knee dated 04/21/2018. 4. My office notes. 5. Operative report from Empire State Ambulatory Surgical Center for arthroscopy of the right knee dated 11/15/2019. History: The patient is status post motor vehicle accident where she was the driver of the car. She was struck by a truck. She noted immediate pain in the right knee with difficulty walking and standing. She noted weakness. She denies any pervious history of pains or injuries to the right knee prior to this accident. She underwent physical therapy without relief. She was initially seen by Dr. Goldenberg and was treated with physical therapy and pain management treatments. She was then sent to me for orthopedic evaluation of the right knee on 07/12/2018. She is complaining of persistent pain in the right knee with locking, catching and giving way. She noted worsening symptoms despite physical therapy. She was currently at that time, doing desk work. Physical Examination: Initial Evaluation Right Knee: Examination of the right knee revealed positive patellofemoral grind test. Positive patellar apprehension test. She has medial tenderness. Positive McMurray sign. Range of motion is 0-110 degrees, normal is 0-140 degrees. She has swelling. She has catching, popping, and locking of the right knee. Review of MRI scan taken of the right knee revealed medial meniscus tear with popliteal cyst and partial ACL tear and joint effusion. Personally reviewed MRI report films. Rule out any degenerative/pre-exiting conditions of the Right Knee. FILED: BRONX COUNTY CLERK 11/03/2021 07:41 PM INDEX NO. 35123/2019E NYSCEF DOC. NO. 39 RECEIVED NYSCEF: 11/03/2021 Page 2 July 26, 2021 RE: Thompson, Tykima Date of Accident: 03-26-2018 Given the fact she has worsening symptoms of the right knee with mechanical symptoms, locking, and catching, and severe pain, I would recommend she undergo arthroscopy of the right knee. This was performed on 11/15/2019, intraoperative findings included a medial meniscus tear of the right knee with synovitis and chondral injury. She underwent arthroscopy of the right knee and partial medial meniscectomy, synovectomy, and chondroplasty of the patellofemoral joint. Postoperatively, she underwent physical therapy. She did sustain injuries to her neck and back and right wrist. She was treated by Dr. Goldenberg for these injuries. Currently, she states she notes persistent pain in the right knee. She denies any subsequent accidents to the right knee. She denies any previous history of pain or injuries to the right knee prior to this accident. She notes persistent pain and difficulty kneeling, descending stairs, sitting for prolonged periods of time, she is currently not working. Physical Examination: Current Right Knee: Examination of the right knee reveals positive patellar grind test. Positive patellar apprehension test. Range of motion is 0-115 degrees, normal is 0-140 degrees. She has mild swelling. Knee is stable. Skin is intact. Neurovascularly intact. I used a Goniometer to measure ROM. Impression: Right knee meniscal tear with chondral injury and status post arthroscopy, status post motor vehicle accident with persistent pain and stiffness. Recommendations/Plan: It is my medical opinion the injuries and right knee Arthroscopy necessitated sustained to the right knee is related to motor vehicle accident of 03/26/2018 appears to be sustained permanent structural damage with significant limitation to the right is related to this accident. Further treatment options for the right knee included continued therapy, medicine, and injection and repeat surgery with repeat arthroscopy. Pursuant to CPLR Rule 2106.
