Defendant's March 12, 2019 letter indicates that plaintiff's Bill of Particulars was not
supplemented for the following surgeries: March 28, 2014 lumbar spine fusion and June 3, 2015
right knee arthroscopy surgery, however that is untrue.
Regarding plaintiff's March 28. 2014 lumbar spine fusion: plaintiff served a supplemental
bill of particulars claiming such surgery on or about April 22, 2014. At such time, plaintiff also
served a Further Exchange of Medical Records providing the respective medical records and an
authorization to obtain the same.
Regarding plaintiff's June 3, 2015 right_knee arthrosconv: plaintiff served a supplemental bill
of particulars claiming such surgery on or about November 12, 2015. On or about February 22,
2016 plaintiff served a Further Exchange of Medical Records providing the respective medical
records and an authorization to obtain the same.
A courtesy copy of plaintiff's aforementioned pleadings are annexed hereto for your review. To
avoid further delay of this matter, fresh, HIPAA compliant authorizations to obtain plaintiff's
medical records pertaining to all of the aforementioned surgeries are enclosed.
It is plaintiff's belief that she is in full compliance with the April 29, 2019 Order of Honorable
Laura G. Douglas.

epidural lead via T12-L1 percutaneous translaminar approach; 3) placement of thoracic epidural

June 3, 2016 Lumba_r.Spine Hardware Removal Surgery:
Pre-Operative Diagnosis: 1) chronic back pain, left-sided; 2) painful hardware L4-Si ; 3) broken
hardware, left S1 screw; 4) status post L4 to S1 fusion, solidly healed;
Post-Operative Diagnosis: same as pre-operative diagnosis;

November 13, 2015 Cervical Spine Fusion Surgery:
Pre-Operative Diagnosis: 1) adjacent level disc herniation, annular tear, C4-5 and C5-6; 2)
debilitating neck and occasional right greater than left arm pain; 3) status post previous C6-7
fusion; 4) history of disc herniation, annular tears secondary to auto accident, C4 to C7;
Post-Operative Diagnosis: same as pre-operative diagnosis;
Procedure: 1) C4-5 and C5-6 anterior cervical discectomy and fusion using PEEK interbody
cage, divergence cages and divergence anterior cervical plates with complete decompression of
posterior osteophyte complexes; 2) right iliac crest bone marrow aspirate for concentrated stem
cells;

We next made a pocket for the generator in the subcutaneous space with knife
followed by Hovie cauteiintion to carefully make the pocket The leads were
