5. On or about May 2, 2022, said defendant property was seized at 1045 West Mission Boulevard,
Ontario, California, 91761 by officers of the Ontario Police Department Who were investigating
violation(s) of H&S 11378.

6. On 0r about May 2, 2022, at 1045 West Mission Boulevard, Ontario, California 91761 said
defendant property was possessed, acquired, used and/or intended for use, as a result of and/or
