IT IS HEREBY STIPULATED by and between plaintiff Unitrin Advantage
Insurance Company (“Unitrin”) and defendant UGP Acupuncture, P.C. (“UGP”) that:
-UGP releases and waives any claims it has asserted or might ever assert for the
May 9, 2016 collision referenced in the complaint (also known as claim number
C029968NY16).
-In consideration of this release, Unitrin shall discontinue this action, with
prejudice, as against UGP only and make the following payments: $820.29 principal,
$164.06 attorney fee and $105 filing fee.
-UGP shall discontinue any suit seeking to collect on these claims.
-The action shall proceed against the remaining defendants.
