Plaintiff Lauren Ashley Payne (“Plaintiff” or “Mother”) and Defendant Chad Ellis Taylor
(“Defendant” or “Father”) have two minor children together, to wit: Colton Ellis Taylor, a male,
born in the year 2010, and Cache Elliott Taylor, a male, born in the year 2017 (hereinafter
referred to as “minor children”). Plaintiff and Defendant have never been married. An Order and
Decree of Parentage was issued in the Fifth Judicial District Court in and for Washington County,
State of Utah, case number 114500388, on or about May 9, 2011, legally establishing parentage,
custody, and support obligations with respect to Colton Ellis Taylor. As part of the above-
referenced case, the Father seeks to legally establish his rights with respect to Cache Elliott
Taylor.
By agreement of the parties and in consideration of the best interest of the children, it is
HEREBY ORDERED as follows:
REGISTRATION OF FOREIGN CHILD CUSTODY DETERMINATION:
As referenced herein, an Order and Decree of Parentage, Custody, and Support was
entered in the Fifth Judicial District Court in and for Washington County, State of Utah, a courtwith jurisdiction pursuant to the Uniform Child Custody Jurisdiction Act, on or about May 9,
2011, in case number 114500388 with respect to Colton Ellis Taylor, a male, born in the year
2010.
The child-custody determination entered in the Fifth Judicial Circuit in and for
Washington County, State of Utah, is hereby registered pursuant to O.C.G.A. § 19-9-85, and is
hereafter enforceable in the same manner as a child-custody determination issued by a Court of
this state.
LEGITIMATION:
By consent of the parties, Cache Elliott Taylor, a male, born in the year 2017, is declared
to be the legitimate child of the Defendant, Chad Ellis Taylor, capable of inheriting from the
Defendant and the Defendant from the child in the same manner as if born in lawful wedlock.
Father has acknowledged paternity of Cache Elliott Taylor, and the Father is in agreement with
all conditions concerning custody and child support provided herein.
CUSTODY:
The parties shall have joint legal custody of the minor children, with the Mother serving
as the primary physical custodian as provided in detail in the Permanent Parenting Plan attached
hereto and incorporated herein.
CHILD SUPPORT:
The parties shall comply with all terms and conditions of the Child Support Addendum,
attached hereto and incorporated herein.
CHILD SUPPORT ARREARAGE:
The Father shall make payment to the Mother for child support arrearage in the amountof $1,500.00 is to be paid as outlined in the Child Support Addendum, attached hereto and
incorporated herein.
ATTORNEYS FEES
Each party shall pay their respective attorneys fees and expenses of litigation.
