COMES NOW, the Plaintiff, SALLY SIMON, as Personal Representative of the
Estate of Michael Simon, by and through her undersigned attorney, and in response to the
Request for Production propounded to said party by Defendants, THOMASZ K. WAZNY,
M.D., and THOMASZ K. WAZNY, M.D., P.A., on February 22, 2000, states as follows:
1.
Response to Request No. 1, photos will be made available for inspection at
a mutually convenient time and place.
Response to Request No. 2, (See Response (q), Plaintiff's Response to
Request to Produce of Defendant, Khalidi, M.D., dated March At,
2000).
Response to Request No. 3, Plaintiff has previously produced those records
in her possession. (See Response (p), Plaintiffs Response to Request to
Produce of Defendant, Khalidi, M.D., dated March [x , 2000).
Response to Request No. 4, Plaintiff's counsel is in possession of certain x-
ray films taken of the decedent at various times during his hospitalizations.
These are apparently duplicate x-rays which were obtained from St. Joseph’s
Hospital and defense counsel should subpoena copies from said health care
facility.
Response to Request No. 5, none.
Response to Request No. 6, objection, work product.
Response to Request No. 7, none other than as previously produced herein.
