1. The Plaintiff, AVIS, is entitled to a Declaratory Judgement against the Defaulting

PHYSCIAL THERAPY REHABILITATION P.L.L.C. that AVIS is not obligated to
provide any coverage, reimbursements, or pay any monies, sums, or funds to any of
the Defendants named herein for any and all No-Fault related services for which
claims/bills have been, or may be inthe future be, submitted by the Defendants to the
Plaintiff, by or on behalf of MITHERLINE PERICLES OR TONY BEAUVIS.
2. That Defendants MARIE DESROSIERS a/k/a MERIE DESROSIERS
MITHERLINE PERICLES and TONY BEAUVIS made material misrepresentations
of fact and false and/or fraudulent statements in the presentation of the claim(s) to
AVIS with regard to the alleged automobile incident of January 17, 2019, and as such
are not entitled to coverage under the subject policy of insurance or any of its
endorsements or amendments thereto.
3. That AVIS is not obligated to provide any first-party coverage, reimbursements, or
pay any monies, sums, or funds, including, but not limited to, Personal Injury
Protection (PIP) or No-Fault Benefits, Additional Personal Injury Protection (APIP),
Supplementary Uninsured/Underinsured Motorists Coverage (SUM), Collision, and
Optional Basic Economic Loss (OBEL) coverage, to any of the Defendants in
connection with AVIS claim number 198003738.
4. AVIS is not obligated to provide any coverage, reimbursement, or to pay any monies,
sums or funds to any of the Defendants for first-party No-Fault Benefits for any
healthcare services rendered to MARIE DESROSIERS a/k/a MERIE DESROSIERS
MITHERLINE PERICLES and TONY BEAUVIS in connection with the subject
loss;
5. AVIS is not obligated to afford any bodily injury coverage, defense or indemnity to
any and all of the Defendants for the subject loss; and
6. AVIS is not obligated to afford property damage coverage to any of the defendants in
the adverse vehicle for the subject loss.
ORDERED, that Plaintiff is directed to serve a copy of this Order with Notice of Entry against all
defaulting Defendants parties via regular mail within thirty (30) days.
