                 Denise   Ray,  being  duly sworn,   deposes  and  says:



                    1.  I am  the Collection              of the Plantiff         and  as        am  f-m-      with all the busmess
                                              Manager,                    herein,         such,
                 records  of the plaintiff and  the proceedings    pe-'a'aiac  to the claim. The   records  referred to herein  are

                 maintained    in the ordinary  course  of p'e±ª'''s  business   and itis in the regular  course  of plaintifPs

                 business   to maintain  such  records.  The  individual(s)  making   entries into  these records  was  under  a

                 business   duty to do so  and  the entries were  made   at the time  of the transaction  recorded   or shortly
                 thereafter.



                    2.  Plaintiff is a Federally-Chartered    Credit Union    licensed  to conduct  business   in the State of New
                 York.



                    3.  The  venue  of this action  is based upon   the Defendant's   rasidance  and/or  place  of transaction  in the

                 County   of Chenango    in the  State of New   York.



                    4. Upon   review  of plaintiffs  records,  there isdue  and  owing   to the Plaintiff by the  Defendant(s)    the

                 principal  sum  of $11,638.47,    accrued  but  unpaid  interast as of October   1, 2020  in the sum   of $1,458.52,
                 accumulated    late charges  and  miscellaneous    fees of $1,951.86,   for a total amount   due  of FIFTEEN

                 THOUSAND        FORTY      EIGHT    AND    85/100  ($15,048.85)   plus  interest at the rate of 9.190%   from

                 October   1,2020,   reasonable   semay     fees ifprovided   by the  contract and  allowed   by the  laws of the  State

                 of New    York,  by reason  of the Defendant(s)    default under  the  terms  of a Loan  and  Security  Agreements

                 and  Disclosure   Statement;   less payments   received  subsequent    of the ce==cement         of thisaction.



                    5.  There  is further due  and owing   to Plaintiff(s) reasonable   attorney's fees, ifprovided    by the  contract

                 and  allowed   by the laws  of the  State of New   York,  and  with  the costs and  disbursements    of this action,
                 less payments    made.



                    6.  Counsel   for your deponent   has  advised  that this action  was  commenced     by the  filing of a Summons

                 and  Comp!•int    (notice) with  the County   Clerk's  Office  and Dafand-E4s)     has  not appeared  and  has  not

                 answered    herein.

                 WHEREFORE,         Plaintiff demands   judgment    against  the Dafend-(s),     for the total emount   claimed   due

                 herein  together  with reasonable   attorneys  fees, ifprovided   by contr    t and allowed   by  law and  the costs

                 and  disbursements    of this action, and  whatever   Edif::a|relief    the  Court  deems  just, proper  and

                 equitable,  less payments   made.
