records of Plaintiff.
3) Your Affiant has reviewed the accounts receivable file in the above-referenced case and the
Defendant owes the Plaintiff the sum of $12,380.13. This balance is subject to increase due to the on going
accumulation of delinquency charges and interest as provided for by contract.
4) The Defendant became delinquent on this account on or about 09/18/03.
5) Defendant’s agreement with Plaintiff provides that Defendant shall be responsible for reasonable
costs of collection, including attorney's fees.
