Now comes the Petitioner, Stanislaw K. Chrobak respectfully
shows unto this Honorable Court as follows:
1. That the Petitioner, STANISLAW K. CHROBAK is 60 years of
age, was domiciled and resided in the County of Cook and State of
Illinois , and has maintained said domicile and residence for at
least (90) days preceding the finding of this Court and that he is
presently residing in the County of Cook and State of Illinois,
presently resides at Chicago, 43 N.May St. Apt 2A Addison, IL
60101. Petitioner is employed.
2. That the Respondent, HELENA K.CHROBAK is 58 years of age,
and presently, resides in Poland, Rosielnica 10,36-220 Rosielnica
3. The parties were married on July 4, 1981 and said marriage
was registered with Bureau of Vital Statistics in Jasienica
Rosielna Poland.
4. That as result of marriage five children were born to the
parties, namely: .
TOMASZ CHROBAK born on 04/20/1982
ARKADIUSZ CHROBAK born on 02/16/1984
MONIKA CHROBAK born 01/19/1987
JUSTYNA CHROBAK born 04/21/1989MALWINA CHROBAK born 09/01/1991
no children were adopted during this marriage, and the Respondent
is not now pregnant.
5. That there existing grounds for Dissolution of Marriage within
the meaning and purview of the Illinois Marriage and Dissolution
of Marriage Act, 750 ILCS 5/401 (a) in that the parties are
separated and have lived separate and apart for continuous period
since 12/20/2011 and irreconcilable differencés have caused the
irretrievable, breakdown of the marriage; the parties efforts at
reconciliation have failed and future
6. All marital property has been divided.
7. That the Petitioner has proved the allegations of his petition
for dissolution of Marriage which are completely sufficient under
Illinois law on which to base a Judgment for Dissolution of
Marriage, and that he is entitled to Judgment for Dissolution of
Marriage.
7.That , the Petitioner has proved the allegations of his
petition for dissolution of Marriage which are completely
sufficient under Illinois law on which to base a Judgment for
Dissolution of Marriage, and that he is entitled to Judgment for
Dissolution of Marriage.
WHEREFORE, the Petitioner prays as follow:
A. That a Judgment for Dissolution of Marriage be entered in
favor of both parties dissolving their marriage.
