Shannon Jones, an attorney duly admitted to practice before the courts
of this state, affirms the following under penalty of perjury that:
1. Affirmant is an Assistant Attorney General of the State of New
York, and of counsel to the Hon. Letitia James, Attorney General of the
State of New York, the attorney for the plaintiff, is fully familiar with
the facts of the circumstances of this action, and is authorized pursuant
to CPLR §3215(f).
2. This action was commenced by the filing of the summons with notice
upon defendant(s) per the annexed proofs of service, and is an action to
recover the sum of $28,071.80 for services rendered to the defendant(s) set
forth in the summons with notice previously served in this action, which is
made a part of this submission to the clerk.
3. The additional notice of default required by CPLR 3215 (g) was
mailed to the defendant(s) on May 18, 2021, which date is at least twenty
(20) days prior to the entry of this judgment. Proof of service thereof is
annexed hereto.
4. The basis for venue for this action are the residence of the
plaintiff and the county in which the cause(s) of action arose.
WHEREFORE, affirmant demands judgment against the defendant(s) in the
sum of $46,657.31, which includes interest from September 7, 2014, and the
costs and disbursements of this action.
