On Novernber 18, 2020, | completed a comprehensive psychiatric assessment of Mr. Christopher
Pleasant at my private practice office, Elite Medical Welinéss {EM\V} in Lake Charles, Loulsiana. The
overall assessment* hashed approximately 123 minutes: Thig assessment included a cailection of vital
signs, which took about three minutes, fallowed by the completion of four self-report psychometric
measures, which took Mr. Pleasant roughly 37 minutes, Thereafter, | engaged him in a direct Interview,
which lasted about86 minutes, After the interview, a licensed psychiatric technician in the clinic, Ms.
