ATTENDING NOTE
The patient returns today to discuss his MRI and to finalize treatment plans. The patient underwent a
MRI of his left foot to rule out deep invasion of a perSistent squamous cell carcinoma arising in the
interdigital space between his fourth and fifth toe on his left foot.This reveals extensive involvement
of the fifth digit however no evidence of bony involvement of lh e fourth and no evid ence of dee p
infiltration of the tumor.I discussed this with Mr. Valenti and his pa rtn er who accompanies him tOd ay.
I have recommended resection of the fifth toe and possible resection of the fourth depending on our
intraoperative findings .Reconstruction will either take place a t the lime of surgery or in a delayed
fashion should this be more appropriate based on the extent of resectio n and lhe wou nd following
removal. We plan intraoperative frozen section analysis of a questionable margin. If there is concern
about a margin we will p roceed with delayed reconstruction. I have discussed this at length with Mr.
Valenti. W e have discussed the anticipated potential complications of infection, disability, pain with
walking, inability to wea r normal shoes, swelling, edema and prolonged recovery associated with foot
surgery. All of his questions w ere answered to his satisfaction and he wishes to proceed. We will
proceed as outlined on 09/20/2010.
