FILED: KINGS COUNTY CLERK 10/26/2020 12:21 PM INDEX NO. 501262/2019 NYSCEF DOC. NO. 18 RECEIVED NYSCEF: 10/26/2020 HON. RICHARD J. MONTELIONE Acting Justice of the Supreme Court Kings County Courthouse 360 Adams Street Brooklyn, NY 11201 718-500-4012 (chambers) 212-952-6286 (fax) Dear Counselors: Please be advised that there is an upcoming inquest scheduled for this matter. Beginning October 27, 2020, Part DJMP will begin conducting scheduled inquests via Microsoft Teams. Attorneys with cases on the calendar who are ready to proceed must contact Ms. Ting Yan Wu, Esq., Judge Montelione’s Court Attorney, at twu@nycourts.gov to schedule a date and time for the inquest via Microsoft Teams. Inquests will be scheduled on a first-come, first-serve basis on the Tuesday and Thursday mornings of the week originally scheduled (for example, if the inquest is calendared for October 27, 2020, inquests may be scheduled to take place commencing at 10:30 on October 27 or October 29). In the email, the subject headline must state “Index Number, Name of the Case, ‘Inquest Scheduling Request’.” Requests must be made at least three business days prior to the scheduled calendar date. If an interpreter is required, this must be included in the request. Attorneys with cases on the calendar who are NOT ready to proceed must contact Ms. Stephanie Santo, the Part Clerk, at slsanto@nycourts.gov, to request an adjournment of the inquest. In the email, the subject headline must state “Index Number, Name of the Case, Inquest Return Date, ‘Inquest Adjournment Request’” and state the reason for the request. Parties may assume that the inquest will be adjourned upon submission of the email. This request must also be e-filed. If there is no communication with the court regarding your case, the inquest will be dismissed on default. All evidence, such as medical records, must be pre-marked and emailed to slsanto@nycourts.gov prior to the scheduled Microsoft Teams Inquest meeting (not the calendar date). All emails containing medical records or other confidential materials must be identified as “PERSONAL AND CONFIDENTIAL MEDICAL RECORDS.” Evidence packet must include a copy of the marked pleadings; all exhibits should be individually named files. Attorneys are encouraged to subpoena their own copy of all records to facilitate this process. Working copies are not required. Certified medical records as required, please see part rules. 1 of 1
