was referred to The Store at New start on 3/1/21. The offender was instructed by this officer to
complete community service hours on 3/12/21, 3/19/21 and 4/9/21. The offender has failed to
complete 4 community service hours per month and has a remaining balance of 150 community service
hours, as of 04/15/21.
Violation of Condition (9) of the Order of Probation, by failing to comply with all instructions given
to him by this probation officer. The offender violated his probation by failing to stay confined to
his approved residence located at 634 Fresno Court Kissimmee, FL 34758. On 4/10/21, this officer
attempted to conduct a home visit at approximately 0710 hours. This officer called the offender's
phone number at 407-433-9028 to instruct him to come to the front door. The offender answered the

he stayed overnight. The offender did not have permission to visit his sister's residence nor to stay
