state and federal constitutional rights available to me and specifically including the RIGHT TO. REMAIN

Arizona, 451 U.S. 477 1880 (1981); McNeil y. Wisconsin, 501 U.S..171 (1991); Michigan v. Jackson, 475
U-S. 625 (1986). invoke all available rights under international treaties and agreements. I hereby express
my unequivocal desire for the assistance of a competent attorney-to deal with all forms of interrogation by
law enforcement agencies and their agents concerning all criminal activity. [demand that my civil rights be
respected and all Jaw: enforcement personnel ahd their agents and, prosécutors aiid their agents avoid
conversing with me concerning any type of criminal activity whatsoever unless my attorney is present.
Irefuse to hereafter waive any constitutional in the absence of my attorney, Anyone hereafter violating my

d. Consent to any polygraph test or voice stress examination, or the giving of any Voice sample,
