personal knowledge, deposes, and says:
1 Affiant has access, custody and control of the records maintained with respect to the mortgage loan
account.
2. Affiant is familiar with the transactions between the parties hereto and has personal knowledge of the
books and records of the Plaintiff insofar as the matters stated in this affidavit are concerned
3, Affiant is familiar with the business practices of Plaintiff and it is the regular business practice of
Plaintiff to maintain a paper and computer record (the “Loan Records”) of acts, transactions, payments,
communications, escrow account activity, disbursements, events, and analyses with respect to the mortgage
loans that Plaintiff owns or services, .
4, The Loan Records are maintained in the ordinary course of its regular business activity.
3 Affjant has personal knowledge of the facts herein set forth, or has based his or her knowledge on the
business records of the Plaintiff.
6. On December 11, 2013, a letter was sent to AMANDA SHEETS, Defendant in this action and executor
of the subject Note and Mortgage, notifying him of the status of the Joan, the amount that was past due, and
pursuant to Paragraph 22 of the Mortgage, notice of intent to accelerate should the past due amounts not be
