At your request, a lumbar spine MRI examination performed on Jose Fernandez was reviewed.
The examination was performed at Dynamic Medical Imaging, PC on 7/27/2018. This is one
month and 12 days following the incident of 6/15/2018.
LUMBAR SPINE - MRI
A lumbar spine MRI examination was reviewed. Sagittal and axial images were provided.
Multiple pulse sequences were obtained.
There is straightening of the normal lumbar lordosis, which is likelypositional and related to the
patient's comfort level during the examination. There is no fracture, subluxation or paravertebral
soft tissue swelling. The vertebral bodies maintain normal height and signal. There is a probable
hemangioma in the T12, L2, L3 and L4 vertebral bodies, which is a non-traumatic finding. The
intervertebral discs maintain normal heights and signals.
L1-L2 is unremarkable. L2-L3 demonstrates a mild disc bulge with mild mass effect on the
anterior thecal sac. The neuroforamen are patent. L3-L4 demonstrates a mild subligamentous
disc bulge without mass effect on the anterior thecal sac. The neuroforamen are patent. L4-L5
demonstrates a mild disc bulge with mild mass effect on the anterior thecal sac. The
neuroforamen are patent. L5-S1 demonstrates a mild disc bulge with mild mass effect on the
anterior thecal sac. The neuroforamen are patent. Disc bulging has no traumatic basis. Itis
degenerative in origin, related to ligameñtous laxity and weakening of the outer ligamentous
fibers.
The conus medullaris terminates at approximately T12-L1. The cauda equina is unremarkable.
The paraspinal musculature is intact.

RIGHT KNEE - MRI
A rightknee MRI exa_mination was submitted for review. Sagittal, axial, and coronal images
were provided. Multiple pulse sequences were obtained.
There is no fracture or dislocation. The distal quadriceps and patellar tendons are intact. The
anterior cruciate and posterior cruciate ligaments are intact. The medial collateral ligament and
lateralcollateral ligament complexes are intact. There is no bone marrow edema. The anterior
horns, bodies and posterior horns of the medial and lateral menisci are intact. There is no soft
tissue swelling. The surrounding musculature is intact. There is no popliteal cyst.
IMPRESSIONS:
> No fracture, dislocation or internal derañgement of the knee.
> No posttraumatic changes causally related to the 6/15/2018 incident.

RIGHT ANKLE - MRI
A right ankle MRI examination was submitted for review. Sagittal, axial, and coronal images
were provided. Multiple pulse sequences were obtained.
There is no fracture or dislocation. There is no bone marrow edema. There is no soft tissue
swelling. The surrounding musculature is intact. There is an ankle joint effusion, which is a
nonspecific transient process that will resolve without sequelae. The subtalar joint is
unremarkable. The ankle mortise is unremarkable. The Achilles tendon is intact. The plantar
fascia is intact. There is a small spur at theinsertion of the Achilles tendon and origin of the
plantar fascia, which is a chronic bony productive change that could not have occurred in the
time interval between the incident and the examination. There is mild pes planus, which is not
related to trauma. Fluid distends the tendon sheath of the flexor hallucis longus compatible with
tendinosis, which is a chronic finding related to repetitive motion that could not have occurred in
the time interval between the incident and the examination. There is no tear. The remainder of
the flexor and extensor tendons are intact. The peroneal longus and brevis tendons are intact.
The ligaments about the ankle including the anterior and posterior tibiofibular ligaments,
talofibular ligament, deltoid and spring ligameñts are all intact. The surrounding musculature is
intact.

examination of 7/27/2018.

patient's comfort level during the examin ation. There is no fracture, subluxation or paravertebral
