           YOU    ARE    HEREBY       SUMMONED       to   appear       in   this    action      by    serving       a  notice      of
           appearance         on    plaintiff's         attorney        within      twenty.      days    after.the         service       of
           this     summons,        exclusive        of.  the    day    of    service,       or   within      thirty       days    after
            service      is    complete       if   this     summons       is    not   personally         delivered         to   you
           within       the    State     of   New    York.


           TAKE     NOTICE      that     the    nature      of   this     action      and    the    relief      sought       is   to
           recover       damages       for    Damage      to    Guide     Rails        ,  NYS    Thruway      Authority         as   a
           result       of   a  motor      vehicle       accident       occurring         on   or   about     April      29,    2019,
            for   the    principal         sum    of   $2,753.83,         with     accrued       interest       in    the   amount       of
           $1,027.41,          as   well     as   interest       from     November        5,   2022,     a   collection         fee
           pursuant        to   State      Finance       Law    Section       18   in   the    amount      of   $809.41,        plus     the
            costs     and    disbursements           of   this     action.


            PLEASE      TAKE    FURTHER       NOTICE      that,      in   case     of   your     failure       to   appear,
            judgment       will     be   entered       against       you    by   default       for    the    relief      demanded        in
            the   notice       set    forth     above,      together        with     interest,        costs,      disbursements
            and    fees    authorized         by   law.


            Plaintiff        designates         Albany      County      as    the   place      of   trial.        The    basis      of
           venue      is   that     the    plaintiff        resides         t  200    Southern        Blvd.,      Albany,       NY
            12201-0189.
