MARIE JEAN BAPTISTE, hereby being duly sworn, states under oath the following:
1. That I am the plaintiff in the above captioned matter.
2. I reside at 710 East 240 Street, Bronx, NY 10470
3. The accident occurred on November 26, 2018 at about 2:10 pm at or near 1204
Flatbush Avenue, Brooklyn, NY. I was a passenger in a motor vehicle with New York plate
number 44436BB. I saw this plate at the scene. I gave this plate number to the police. The
motor vehicle owned by MVP IMPERIAL CORP. and the bus driven by defendant ONEIL A.
LEVY and owned by the defendant NYC Transit Authority collided with each other. This
accident was caused by the negligence of the defendants.
4. As a result, I required medical treatment and care.
5. I therefore ask that this court grant the within motion.
