I am a Board Certified Orthopedic Surgeon and have been asked to perform an
orthopedic examination on 06/27/2019 regarding Ms. Frire Clavijo at the Richmond Hill,
New York office. The claimant's valid photo Identification was checked and verified. A
chaperone was present during this examination.
ACCIDENT HISTORY:
The history was obtained from Ms. Frire Clavijo who reports that she was involved in a
motor vehicle accident on 12/29/2017. The claimant reports that she was a restraiñêd
rear-seat passenger when the vehicle was allegedly struck on the front.
Ms. Frire Clavijo denies any loss of cõñsciousness fal!cwiñg the accident.
Ms. Frire Clavijo reports that she was transported via ambulance to the Elmhurst
Hõspital Center that same day for evaluation. She raparts that she was then admitted to
the hospital for one day for further observation. She was re!eased.
Ms. Frire Clavijo reports that she did not sustain any fractures or lacerations,
Following the accident, Ms. Frire Clavijo came under the care of various physicians,
She was Initiated on a course of conservative management which included physical
thêrapy, chiropractic care, acupuncture treatmênt, massage therapy, heat treatment, and
ice treatment. The treatment began at a frequency of three to four times a week. She
reports that the treatment received has not been helpful. She reports that she does not
continue to receive treatment at the present time. She reports that she had MRI of the
neck and back.
Ms. Frire Clavijo reports that she required the use of crutches as a resultof the sccident.
She underwent right knee surgery on 04/30/2018.
CURRENT COMPLAINTS.
Ms. Frire Clavijo reports that she has complaints of pain in her neck, mid back, right hip,
and right knee.

PRIOR MEDICAUSURGlCAL HISTORY:
Ms. Frire Clavijo did not d½c!êss any details regarding her past medical history.
Ms. Frire Clavljo denies any prior or subsequent accidents or sustainiñg any injuries.
TREATMENT HISTORY:
Ms. Frire Ciavljo does not report undergoing any surgeries in the past.
CURRENT MEDICATIONS:
Ms. Frire Clavijo is currently taking Naproxen, and Tylenoi and Advil.
Ms. Frire Clavijo denies taking any medications today prior to thisexamination.
EMPLOYMENT HISTORY:
Ms. Frire Clavljo reports that she was empicyed on a full-time basis as a waitress at the
time of the accident. She reports that currently she is not working.
REVlEW OF SUBMITTED MEDICAL RECORDS:
The following medical records were available for my review:
1. Supp!êmental verified billof particulars, index no: 703456/2018.
2. Police accident report dated 12/29/2017.
PHYSICAL EXAMINATION:
Ms. Frire Clavljo is a 30-year-old right-handed female. Her reported height is 5 feet tall
and her reported weight is 165 pounds. She has brown hair and brown eyes.
RANGE OF MOTION MEASUREMENTS:
The values of allthe measuremêñts were compared to the normal active range of motion
values according to the publication, "Guidelines to the Evaluation of Parmanent
impairment", 5th edition published by the American Medicâi Association. The range of
motion of the examined body parts were performed by the claimant. This is a subjective
maneuver on the part of the claimant. Measurements of the ranges of motion were
perfcrmed by the examiner using a hand-held goniometer. The measurement itself is,
therefore, an objêctiva measurement of the claimant's subjective efforts.
ORTHOPEDIC EXAMINATION:
Cervical Spine: There is no muscle spasm upon palpatian over the paracervical
muscles. There is no complaint of tendemess upon palpaticñ over the paracervical
musdes. Range of motion reveals flexion at 50 degrees (50 degrees normal), extension

at 60 degrees (60 degrees norma|), right lateral flexion at 45 degrees (45 degrees
normal), left lateral flexion at 45 degrees (45 degrees normal), right rotation at 80
degrees (80 degrees normal), and leftrotation at 80 degrees (80 degrees normal).
• Distraction - negative.
• Compression - negative.
• Jackson's - negative.
• Soto-Hall - negative.
Neurological examinaticñ of the bilateral upper extremities shows as follows: There Is no
aticphy. Muscle strength is at 5/5. Deep tendon reflexes are at 2+. Sensation to light
touch is intact.
Thoracic Spine: There is no muscle spasm upon palpation over the parathcracic
muscles. There is no complaint of tenderness upon palpation over the parathoracic
muscles. Range of motion reveals flexion at 45 degrees (45 degrees normal), extension
at 0 degrees (O degrees normal), rightlateral flexion at 45 degrees (45 degrees normat),
leftlateral flexion at 45 degrees (45 degrees normal), rightrotation at 30 degrees (30
degrees normal), and teftrotation at 30 degrees (30 degrees normal).
Right Hip: There is no heat, swsiting, or redness appreciated. There Is no complaint of
tenderness upon pa!pet!on. Range of motion reveals flexion at 100 degrees (100
degrees nõrma'), extension at 30 degrees (30 degrees ñórmal), abducticñ at40 degrees
(40 degrees normal), addLdon at 20 degrees (20 degrees normal), exterña: rotation at
50 degrees (50 degrees normal), and intemal rotation at 40 degrees (40 degrees
normal).
• - negative.
Trendelenburg
Right Knee: Examination reveals surgical scar. There is no heat, swelling, effusion,
erythsma, or crepitus appreciated. There Is no complaint of tenderness upon pa!petion.
Range of motion reveals flexion at 150 dsgrses (150 degrees normal), and extension at
0 degrees (O degrees normal).

Tracking
• Stable-Varus/Valgus - yes.

IMPRESSION:

The examinee presents with a normal orthapsdic examination on allobjactive testing.
The orthopedic examination is objectively ñormal and indicates no findings which would
result in orthopedic limitations in use of the body parts examirisd. The examinee is

as well as usual daily activities Including regular work duties.
