1. The Plaintiff is a State-Chartered Credit Union, licensed to conduct business in New York, with a
principal place of business at 231 Hill Road , Rome, County of Oneida, State of NY.
2. Upon information and belief, the Defendant(s) is/are an individual(s), with a residence at 241 Medford
Rd. Syracuse, NY 13208 and 8342 Nates Lane Cicero, NY 13039.
3. That there is due and owing to the Plaintiff by each Defendant, the sum of $4,767.92, plus interest at the
rate of 2.490% from 09/21/16, plus reasonable attorneys fees to the extent provided by the contract and
allowed by the laws of the State of New York, by reason of Defendants default in payment for Note and
Disclosure Statement
4. That although payment has been demanded of the Defendant(s) by the Plaintiff, such has been neglected
and refused.
WHEREFORE, Plaintiff demands judgment against the Defendant(s) in the sum of $4,767.92, together with
interest at the rate of 2.490% from 09/21/16, reasonable attorneys fees, if provided by contract and allowed
by law and the costs and disbursements of this action, less ents made.
