Plaintiffby and through itsattorneys, complaining of the defendant states:
1. At alltimes mentioned herein, plaintiffand judgment creditor was and remains a public welfare department
located in the County of Lewis and State of New York.
2. On information and belief,defendant and judgment debtor resides in the County of Lewis and State of New York.
3. On or about September 21, 2011, judgment by default, Index No.: CA2010-000629 was duly entered in favor of
thisplaintiff and against thisdefendant for the sum of Two hundred twenty dollars and 83/100 ($220.83) in the Lewis
County Clerk's Office. A copy of said judgment isannexed hereto and marked Exhibit "A". No part thereof has been
paid.
4. To date,no payments have been received, leaving the judgment balance remaining due of $220.83 plus interest.
5. Said judgment was duly docketed on September 21, 2011, in the Lewis County Clerk's Office, more than nine
years prior to the commencement of this renewal action.
6. This action isbrought under CPLR 5014(1).
WHEREFORE plaintiff demands the entry of a renewal of the existing judgment in favor of the plaintiffand against
the defendant for the sum remaining due on said judgment with interest,together with costs and disbursements of this
action and such other, further and different relief as to thiscourt may seem just.
