for said Defendant to appear, answer or raise an objection to the complaint in point of law having
expired, and said Defendant not having appeared, answered or raised an objection to the
complaint in point of law, and the notice required by the last paragraph of CPLR 308 having
been mailed, December 16, 2010.
NOW, upon summons and complaint and proof of service thereof, the notice required by
the last paragraph of CPLR 308 and proof of mailing thereof, the affidavit of the appropriate
Lewis County Department of Social Services personnel sworn to on the 21st day of September,
2011, and upon motion of KEITH A. LYNDAKER, Attorney of Record for the Plaintiff, itis
ORDERED, ADJUDGED AND DECREED, that the Plaintiff, LEWIS COUNTY
DEPARTMENT OF SOCIAL SERVICES, residing at 5274 Outer Stowe Street, Lowville, New
York, do recover of the Defendant, DAVID LUDWIKOWSKI, residing at 7052 West Main St.,
Port Leyden, NY 13433 the sum of $527.24, with interest from September 20, 2011, plus costs
and disbursements of this action and that Plaintiff have execution therefor.
