Defendant(s), respectfully alleges upon information and belief.
1. Plaintiff is a National Bank located in Utah.
2. That upon information and beliefthe Defendant RYEDELL DAVIS AKA RYDELL L DAVIS
is a resident of the county where thisaction isbrought.
3. That upon information and beliefthe Defendant THA CREW LLC is a business entity organized
and existing under the laws of New York State.
4. That the Defendant(s) entered into credit agreements ("Agreements") with the Plaintiff consisting
of accounts in the name of the Defendant(s) as follows:
account ending in 3008 with a balance of $1,011.12
account ending in 3008 with a balance of $10,693.64
account ending in 3005 with a balance of $14,038.82
5. That the Defendant(s) breached the terms of the aforementioned Agreements.
6. That there isnow due and owing to the Plaintiff from the Defendant(s) as a result of the
aforementioned breach of the Agreements by the Defendant(s), the sum of $25,743.58.
WHEREFORE the Plaintiff,AMERICAN EXPRESS NATIONAL BANK, demands judgment
against the Defendant(s), RYEDELL DAVIS AKA RYDELL L DAVIS and THA CREW LLC, for the
sum of $25,743.58, the costs and disbursements of thisaction, and for such other and further relief as the
Court deems just and proper.
