Site Safety Managers Comments
1.) S. S. M. arrives onsite at 6:00
2.) Walk thru jobsite at 7:05
3.) S. S. M. leaves jobsite at 5:30
4.) S. S. M. walked the perimeter of the site and did a visual inspection of the
concrete barriers with the wood fence on top and found no issues, the plastic
barricades for the pedestrian walkway were moved away on East 29th St. so the
neighborhood residents could park their vehicles in the pedestrian walkway
causing the pedestrians to walk outside the walkway and into traffic.
5.) Prestige Construction onsite for site supervision, light carpentry,
housekeeping, gate keeping, flagging, moving material, fixing street
barricades.
6.) Braga Corp. onsite for foundation work, receiving concrete delivery, concrete
pouring of footings and wall, rebar fabrication and installation, form
installation, moving material.
7.) Almeida Concrete Pumping onsite to pump concrete into site.
8.) King Concrete onsite for delivering of the concrete.
9.) Big Apple Testing onsite for rebar inspection.
10.) M T Group onsite for concrete testing.
11.) S. S. M. advised Prestige Construction that the need to have onsite a stamped
Foundation plan in order to avoid a D. O. B. violation.
12.) Reminder: Local Law 196 of 2017: Logs: All permit holders are required to
maintain a daily log that identifies each construction worker and supervisor.
( including Con. Supers, - S. S. M.,- S. S. C.,- C. S. M., - competent person’s )
on the construction site and their proof of training, it is the responsibility for
permit holder to have a D. O. B. log that be provided to the Dept. upon
request.
13.) Reminder: All gates and doors need to swing inwards to the jobsite and not
swing out into the pedestrian walkways.
14.) REMINDER: During construction there needs to be at the minimum of TWO
points of egress with no obstructions as per the D.O.B. and F. D. N.Y.
15.) REMINDER: Any worker coming onsite and performing any duties
pertaining to the jobsite must have at the minimum a 10 hour O. S. H. A. card
and then must be job orientated by the S. S. M. to work onsite.
16.) REMINDER: All perimeter fencing must be designed, engineered for wind

17.) REMINDER: All rebar that is exposed standing up to a fall hazard must have
an O. S. H. A. standard cap on or an O. S. H. A. cap that can be adapted to a
2 x 4 to cover a straight length of rebar for protection.
18.) REMINDER: Any trade performing HOT WORKS needs to have the proper
certification to perform the task and take out a HOT WORKS permit for the
task on a daily basis.
19.) REMINDER: REMINDER: All digging and trenching needs to be sloped or
benched.
20.) REMINDER: All workers working by a leading edge, platform or scaffold
must use the proper fall arrest system. (TIE OFF)
21.) REMINDER: All tradesmen using a A-frame ladder must open ladder cannot
step on the two top rungs to use it properly and not lean the closed ladder
against the wall.
22.) REMINDER: Any trade using a grinder, saw or drill must have safety glasses
and guard on their tools in order to use it properly.
23.) REMINDER: All site gates must be kept closed at all times except for
deliveries.
24.) REMINDER: All onsite security personnel need to have a 10 - O. S. H. A. and
a F.D.N.Y F60 fire watch certification.
25.) REMINDER: All trades working with electrical tools need to use a G. F. C. I.
with their tools ( All electrical cords need to be hung up off the ground )
26.) REMINDER: Workers are advised not to work over each other.
