a legal and proper purpose for granting the requested relief, and Respondent having no objection
to the relief sought, it is hereby:
ORDERED that Respondent shall amend the birth certificate of Andrew Fracaro, born
May 14, 1921 in the Town of Roxbury, Delaware County, New York to change the name of the
birth Father from “John Fracaro” to “Giovanni Fracaro” and to change the name of the birth
Mother from “Cristeni Carle” to “Cristini Carli”, and it is further
ORDERED that upon receipt of a certified copy of this Order with notice of entry
thereof, proper application by Petitioner, and payment by Petitioner of all applicable fees, if any,
pursuant to New York State Public Health Law §§4174(1)(b) and 4176(4), Respondent shall
release to Petitioner a certified copy of the birth certificate, as amended, of Andrew Fracaro, born
May 14, 1921 in the Town of Roxbury, Delaware County, New York if the original is currently
on file with Respondent.
