                                           NOTICE OF APPLICATION FOR APPROVAL OF PROPOSED
                                       TRANSFER OF STRUCTURED SETTLEMENT PAYMENT RIGHTS,
                                                    ORDER FOR HEARING, NOTICE OF DEADLINE

                                    TO ANY PROPER OFFICER:

                                    YOU ARE HEREBY NOTIFIED that K. McNinch has entered into a transfer

                           agreement with CBC Settlement Funding, LLC (“CBC”) (the “Transfer Agreement”) in which

                           she agreed to transfer to CBC her rights to receive certain future structured settlement payments.

                           CBC has filed with the Superior Court for the Judicial District of Litchfield at Litchfield an

                           Application for Approval of Transfer of Structured Settlement Payment Rights dated October 31,

                           2016 (the “Application”).       Attached hereto are copies of the following:

                                    (1)      The Application;

                                    (2)      the Transfer Agreement (Exhibit A to the Complaint)

                                    (3)      the Disclosure Statement (Exhibit B to the Complaint)

                                    (4)      the Payee has two (2) dependents: Nathan, 13 years old
                                                                                       Elizabeth, 11 years old
                                    YOU ARE HEREBY FURTHER NOTIFIED

                                    a.     that you or any interested party may support, oppose or otherwise respond to the

                           CBC’s Application, either in person or by counsel, by submitting written comments to the court

                           or by participating in the hearing.

                                    b.          A hearing on the Application will be held at the Connecticut Superior Court for

                           the Judicial District of Litchfield at Litchfield at 15 West Street, Litchfield, CT 06579, on the

                           ____ day of _____________, 2016 at __:__ a.m./p.m.

                                    c.      In order to be considered by the Court, written responses, comments or objections

                           to the Complaint and/or this Application must be filed with the Court no later fifteen (15) days

                           after service of this Notice.

                                 NOW, THEREFORE, by authority of the State of Connecticut, you are hereby

                           commanded to give notice of the pendency of the proposed transfer, the Application, and of the

                           time and place of the hearing thereon upon to the Payee, K. McNinch, the Annuity Owner,

                           Hartford Underwriters Insurance Co., and the Annuity Issuer, Hartford Life Insurance Co. by

                           overnight mail or by lawful service of a true and attested copy of this Order and Notice on or

                           before _________________, 2016.
