REQUEST FOR INDEPENDENT MEDICAL EXAMINATION
Pursuant to Practice Book §13-11, the Defendants, RESA WEARABLES, INC.,
COSTCO WHOLESALE CORPORATION AND DAN DICKSON hereby give notice that
the minor Plaintiff, JANE DOE, submit to an independent medical examination, at the Defendants’
expense, to be performed by ANDREW W. MEISLER, Ph.D. on AUGUST 30, 2022 at
10:00AM at 664 PROSPECT AVENUE, MAIN ENTRANCE, FIRST FLOOR,
HARTFORD, CT 06105.
Dr. Meisler will perform a mental examination of the plaintiff’s post traumatic
stress disorder claim as well as plaintiff’s mental pain and suffering and question her about same.
The plaintiff shall provide defense counsel with all treatment records, notes, memos and
correspondence from the plaintiff’s medical providers treating her for the psychological injuries
alleged in this case in advance of the examination.
