EACH OF YOU ARE HEREBY NOTIFIED THAT THE REGENTS OF THE

UNIVERSITY OF CALIFORNIA claims a first lien in the amount of $12,972.67, pursuant to
21
California Labor Code §3856, upon any settlement or judgment rendered in favor of NUPUR
22
AGRAWAL against DENISE ANNETTE SAVONNE, and any and all third party defendants.
23
This lien consists of medical expenses paid in the amount of $8,503.46 and salary
24
continuation in the amount of $4,469.21. This lien claim is not hereby limited to this
25
amount as the workers’ compensation claim of NUPUR AGRAWAL has not been settled or
26
otherwise adjudicated.

THE REGENTS OF THE UNIVERSITY OF CALIFORNIA claims a first lien in the
28
amount of $12,972.67, plus the amount of any additional compensation benefits that it may

1 become obligated to pay in the future under the workers’ Compensation Laws of California, up
2 to the satisfaction of judgment, consisting of monies paid out for indemnity and medical
3 benefits, and any other benefits paid under the workers’ Compensation Laws of California as
4 well as interest thereof according to law as against defendant DENISE ANNETTE SAVONNE.
5 Lien Claimant THE REGENTS OF THE UNIVERSITY OF CALIFORNIA reserves
6 their credit rights in the underlying workers’ compensation claim as part of said lien claim.
