I, Donelle H. Ratheal, of Ratheal & Associates, P.C., being duly sworn, states as
follows:

1. I represented Respondent, Lance Aaron Busbee, in the above-styled case as his
attorney of record.

2. Petitioner, Barbara Ann Busbee, (now Darragh), is indebted to Respondent in the
amount of $17,988.50 (Seventeen Thousand Nine Hundred Eighty-eight and 50/00 Dollars), not
including statutory interest, for an attorney fees award, over and above all offsets. Statutory
interest of 5.25 % per annum began to accrue as of June 16, 2011.

3. Petitioner is indebted to Respondent in the amount of $1,036.52 for child support
arrearages, not including statutory interest, over and above all offsets. Statutory interest of 10%
per annum began to accrue as of December 13, 2011.

Page 1 of 24. I believe that International Environmental Corporation (“the Corporation”) is
Petitioner’s employer. I further believe that the Corporation indebted to or has property within
its possession or under its control, which is not by law exempt from seizure or sale upon
execution, belonging to Petitioner, Barbara Ann Busbee.

5. I am seeking a continuing garnishment for collection of child support arrearages,
and collection of an award for attorney fees and court|costs. y
