            20     (1)      I am an employee of Drug Abuse Alternatives Center ("DAAC").                   I am the front desk receptionist.

            21     My duties include answering the telephones and greeting people when they arrive at DAAC.

            22     (2)      I am   not  an  authorized   agent   for DAAC, nor       am  I authorized    to accept   any  legal documents     on

            23     behalf of DAAC.

            24     (3 )     I have never been served with or received a Summons and Complaint in the above referenced case.
