other evidences of debits or credits to any deposit account at any time duringthe period from January 1, 2017 to the present for each deposit account in
which you have held an interest at any time during said period. This
includes all business and personal bank or other deposit accounts;
2. All insurance policies (on life, liability, casualty, property, etc.) in force at
any time subsequent to January 1, 2017 to the present, in which you have
held an ownership or beneficial interest at any time during said period,
together with all schedules, riders, amendments and attachments thereto;
3. Originals or legible current photocopies (front and back) of all titles for
vehicles in which you have held any interest at any time during the period
from January 1, 2016 to the present;
4. Copies of all financial statements prepared by or for you setting out any
information on your financial condition, whether independently or in
conjunction with other individuals or entities, for any period ending
subsequent to January 1, 2016;
5. Legible and complete copies of your state and federal income tax returns
together with all supporting forms, schedules or other attachments, for the
years 2016 to present;
6. All safety deposit box keys;
7. Records of asset transfers, assignments, gifts, conveyances, mortgages and
other forms of transfers made by you, occurring at any time between January
1, 2016 to the present;
YOU ARE ORDERED TO NOT PAY OUT, TRANSFER, MORTGAGE,
ENCUMBER, OR MAKE OTHER DISPOSITION OF ANY MONEY OR
