COMES NOW Matthew R. Tarvin, attorney for Plaintiff Eugine Vanbeber, being duly
sworn, deposes and says:

That the defendant resides at 321 East Fry Terrace, Claremore, Oklahoma 74017, and that
the mailing address of the defendant is the same. That the defendant is indebted to the plaintiff
in the sum of $1,900.00 for breach of contract dated January 06, 2016, that plaintiff has
demanded payment of the sum, but the defendant has not paid the same and while a portion of
the original loan has been paid, no part of the amount sued for has been paid.
