LET plaintiff show cause before this Honorable Court, located at 88-11 Sutphin
Boulevard, Room , Jamaica, New York, on the day of
2019, at 9:30 a.m. or as soon thereafter as counsel can be heard, why an Order should not
be granted:
(a) Pursuant to CPLR §1021 dismissing plaintiff's Complaint for
failure to timely substitute; and,
(b) For such other further and different relief as this Honorable Court may
deem just and proper.

other Court. In the interim, sufficient cause appearing, therefrom itis hereby:
ORDERED, that sufficient reason appearing therefrom, that delivery of a copy of
