(1) All regular and inactive status speech-language pathologists, audiologists and speech-language pathology assistants shall renew their licenses on a biennial basis, by December 31st of each odd-numbered year.
(2) The Board shall provide the licensee a license renewal notice sent to the address on file with the Board, which will include the following:
(a) The expiration date of the license;(b) The amount of the renewal fee due; and(c) The number of professional development hours required for renewal.(d) For Speech-Language Pathology Assistants, the license renewal notice will include the requirement for the Assistant to report their current supervising Speech-Language Pathologist.(3) All applications for license renewal must be postmarked or submitted electronically by December 31st of each odd-numbered year, and each licensee must submit:
(a) The renewal application completed in full; and(b) Payment of the non-refundable fee for license renewal.(4) Licensees whose renewal forms are postmarked or submitted electronically after December 31st of each odd-numbered year will be charged a delinquency fee. The board may renew each expired regular or inactive status license upon payment of the biennial renewal fee and the delinquency fee.
(5) A license is not considered renewed until the licensee has complied in full with items 3 and 4 above and a new license certificate with a current expiration date has been issued by the Board.
