(A) The board will maintain separate and
		  combined listings of orthotist, prosthetist, pedorthist, or
		  orthotist-prosthetist license holders along with the license holder's
		  address. The fee for the list of license holders must not exceed the actual
		  cost of duplication and mailing.
(B) Any person may obtain a copy of a
		  licensee list by:
(1) Writing the board at:
			 "Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board,
			 77 South High Street, Sixteenth Floor, Columbus, Ohio, 43215-6108," and
			 submitting a check or money order made payable to "Ohio Treasurer of
			 State" in the appropriate amount.
(2) E-mailing the board
			 at board@otptat.ohio.gov. No fee is required for a request fulfilled
			 electronically.
