Every notice of appeal must be filed by the
aggrieved party within thirty days of the mailing date of the adjudication
order or thirty days from the receiving date of a citation issued pursuant to
section 3737.41 or 3737.42 of the Revised Code. The notice of appeal must be
legible and include the following information:(A) The name and address of the applicant or
